An Emergency Profile
An Emergency Profile is a subset of your PHR which may be accessible online, on a profile sheet, wallet card, or USB thumb drive by family members, emergency personnel, and healthcare providers. Using HealthVault, creating emergency wallet card or profile sheet or sharing access to an emergency profile is relatively simple. E.g.,
Create a Wallet Card.
- Sign In to HealthVault
- Click Emergency Profile.
- (beneath picture and Health View).
- Click the Print wallet card button.
- Check box for Include and emergency access code.
- Enter Myself
- (or other person, e.g., spouse, daughter, who will carry the card for your profile).
- Click the Print button.
- Select your printer, print the ‘card’ and then trim/fold it.
Create a Profile Sheet.
- Sign In to HealthVault
- Click Emergency Profile.
- (beneath picture and Health View).
- Click the Print profile sheet button.
- Check box for Include and emergency access code.
- Enter Myself
- (or other person, e.g., spouse, daughter, who will carry the card for your profile).
- Click the Print button.
- Select your printer, print the ‘card’ and then trim/fold it.
Share the Profile.
- Sign In to HealthVault
- Click Emergency Profile
(beneath picture and Health View). - Click the Share this profile button.
- Check box for Include and emergency access code.
- Enter and retype the email address of the person to receive access to the emergency profile (more than one recipient may be added).
- Add an optional note.
- Click the Send sharing access button.
